Emergency Contact

Emergency contact details will be used to create an account for you to access the FA Matchday app.

Medical Information

Please provide any medical information about your child you think may be relevant. Please answer "None" if there are no medical issues.

Declarations

I give permission for my child to be given first aid/taken to hospital if necessary.
I give consent for my child to be photographed by the club during club events.

Payment options

Payments will be assigned in the Matchday App after this form has been processed by a member of the club.

Registration fee will be £45 which will be due on 1st September and subs will be £15pcm paid in arrears from 1st October. A discounted registration fee will still apply for second child
